Breast mass characterisation on MR Mammography

Fischer Score

Fischer score of 0 and1 points Category I (Negative)
score of 2 points Category II (Benign)
score of 3 points Category III (Probably benign)
score of 4 and 5 points Category IV (Suspicious)
score of 6 , 7 and 8 points Category V (Highly suggestive of malignancy)

MRI BIRADS

Enhancing mass with clear malignant features on morphological assessment ( i.e., a stellate shape or borders with spicules or a rim internal architexture ) irrespective of enhancement kinetics.

BIRADS 5

Enhancing focal mass showing indeterminate or suspicious morphology, but none of the previously mentioned features
if kinetics pattern III , BIRADS 5

if kinetics pattern II , BIRADS 4

if kinetics pattern I , BIRADS 3

Enhancing focal mass showing benign shape and border characteristics

if benign internal architecture BIRADS 2

if suspicious enhancement and kinetics pattern I or II , BIRADS 2/3

if suspicious enhancement and kinetics pattern III BIRADS 4

Combined use of both these parameters is more accurate in lesion characterisation
